Wellness Coaching

Dr. Scott works with both individuals and families to identify, create and sustain conditions that promote a life of thriving and optimal performance. Her coaching program focuses on helping individuals grow into becoming the experts of their own well-being by encouraging personal responsibility, deep thinking, self-discovery and self-efficacy.

Is Your Health Fortified?

To get a sense of your overall state of wellness and that of your family, take a couple of minutes to complete this self assessment. Just select the response that reflects how much you agree or disagree with each statement. After completion of the questions, you will receive a brief analysis of the current state of your wellness aligned with specific initial steps towards creating your Fortified Family Health Plan.

1. I recognize that I alone can control my long-term health and wellness.
2. I get at least 7 hours of sleep at night most nights, and I usually provide myself an environment that allows for restful, restorative sleep.
3. My family has defined what it means to be healthy and well through a unified Family Health Vision.
4. I understand the cost of healthcare services I may need in the future, and I have made financial plans with my advisor to cover the future costs of care-related services that may be needed, such as impairment care, disability care, acute care, and dependent living care.
5. Approximately 70% of annual deaths in the United States are due to chronic disease. (Centers for Disease Control, 2005). I recognize and understand that many types of chronic disease are based on lifestyle choices that are within my control.
6. I usually eat a diet rich in fruits, vegetables, lean meats, complex carbohydrates and simple fats, while minimizing my consumption of sugar, simple carbohydrates, salt, saturated fat and trans-fat.
7. The Centers for Disease Control recommends at least 2 hours and 30 minutes of physical activity each week. I consistently exceed this baseline through walking, jogging, cycling, swimming or other physical activities.
8. I monitor my body’s response to new medications, and I feel comfortable giving honest, clear feedback to my physician if I have any concerns.
9. I understand and recognize the value of a healthy blood pressure, proper weight, safe cholesterol levels, an efficient metabolism and a healthy hormone system.
10. I make use of health monitoring technologies (FitBit, Up, Jawbone, etc.) to make sure that my health metrics don’t fall out of alignment; if they do, I know to contact my physician and share abnormal numbers.
11. I understand the concept of “death with dignity,” and I know the level of care (pain management, aggressive life support, resuscitation, etc.) I’d like to receive at the end of my life.
12. When my physician gives me or a close family member a diagnosis for a chronic disease, I speak him or her to find out the best preventative care and disease management choices that can be made.
13. I have made end-of-life plans for myself, and I have a living will that will allow a selected friend or family member to have power of attorney over me in the event that I’m incapacitated.
14. I know the medical conditions for which I have a genetic predisposition, and I follow preventive and screening guidelines to minimize their risk in my life.
15. My family has effective, standardized ways to share information about hereditary diseases that could impact members in the future.
16. I monitor my body’s response to new medications, and I feel comfortable giving honest, clear feedback to my physician if I have any concerns.
17. Approximately 18% of harmful medication errors are preventable. (Ind. Journal of Psychiatry, 2010) I understand why I need prescribed medications, and I have discussed risks, benefits, side effects and alternatives to prescription medications and the conditions for which they are prescribed.
18. Each family member has a specific plan, based on age and life circumstances, on what actions and beliefs to engage in to embrace the family wellness vision. Through these actions and beliefs, family members can outperform their family health history and achieve their personal vision of wellness.
19. Medical records and critical information for family members are stored in a centralized, secured and easily-accessible way. Records can be accessed at any time from any location by qualified family members.
20. Our family has a personal advocate to ensure accelerated, prompt, safe decision making in times of crises. Our advocate oversees all aspects of our preventative, urgent and crisis care.
21. My family has defined what it means to be healthy and well through a unified Family Health Vision.
22. I know my family health history for mental wellness issues.
23. Our family understands and accepts family members with mental health disorders.
24. We have open conversations, without any stigma, about the signs and symptoms that could indicate mental wellness issues in high-risk family members. We recognize that family members face a higher risk for mental wellness disorders during transitional stages in their lives.
25. I know my current purpose, which helps me to center my life. I recognize and understand the different circumstances, factors and state of mind that can trigger stress in my personal and work life.
26. I routinely engage in a secular or spiritual meditative practice which supports my mental health and reduces stress in my life.
27. I know the harm that can be caused by an over-connection to technologies like smartphones, smart watches, tablets, etc. I recognize when technology is being overused, and I take steps to reduce usage.


The coaching process is grounded in aligning personal goals and values through improving well-being with evidence-based mental, physical and emotional strategies.  Key coaching areas include fitness, nutrition, health risk, weight management, stress management and life satisfaction.

 Carol will assist you in finding very personal answers, allowing you to create your own possibilities. Through a non-judgmental process, Carol works to engage hearts and minds to achieve lasting, sustainable results.


That said, Dr. Carol Scott is merely a catalyst that helps you reach your goals through self-empowerment. Client-originated visions and plans are the ones that stick.
